The House Health and Human Resource committee moves to amend
the bill on page two, section one, line sixteen, following the
word "monthly" by inserting the word "fee";
On page three, section two, line nine by deleting the entire
subsection and inserting in lieu thereof the following:
"
"Patient centered medical home" or "medical home" means a
health care setting that facilitates partnerships between
individual patients and their personal physicians and, when
appropriate, the patients' families and communities. A patient
centered medical home integrates patients as active participants
in their own health and well being. Patients are cared for by a
physician or physician practice that leads a multi-disciplinary
health team, which may include but is not limited to nurse
practitioners, nurses, physician's assistants, behavioral health
providers, pharmacists, social workers, physical therapists,
dental and eye care providers and dieticians to meet the needs of
the patient in all aspects of preventive, acute, chronic care and
end-of-life care using evidence-based medicine and technology.
";
On page four, section three, line twenty-one, following the
letter "(a)" by striking the word, "The" and inserting in lieu
thereof the following:
"As resources are available" and a comma:
On page four, section three, line twenty-one, following the
word "implement" and the comma by striking out the comma and the
words "by July 1, 2010" and the comma;
On page six, section three, line fifteen, following the word
"services" by inserting the following "for the purpose of
implementing this article";
On page six, section three, line eleven, following the word
"funds" by striking out the rest of the subsection and inserting
in lieu thereof, the following:
"appropriated by the Legislature or by retaining a
percentage of the monthly per member per month fee
determined necessary by the Finance Board created in article
sixteen, chapter five of this code in consultation with the
Medical Home Advisory Board created in this article." and
On page nine, section seven, line eight, after the letter
"(b)" by striking out the words "No later than December 1, 2010"
and inserting in lieu thereof the following:
"No later than one year after implementation of the plan".
